Childhood Hepatitis with Mycoplasma Pneumonia

Abstract
Purpose: There are many studies of childhood pneumonia by Mycoplasma pneumoniae but a little reports of hepatitis by it in Korea. The purpose of this study is to know its clinical characteristics and to help management of hepatitis children by it.

Methods: I collected the clinical informations of 153 children who had been diagnosed with mycoplasma pnemonia in the Department of Pediatrics, YongIn Severance Hospital, YongIn, Korea from January 2003 to December 2007 by particle agglutination test kit (antibody titer, ?1:160). Hepatitis was evaluated with serum transaminse (>45 IU/L). Clinical characteristics such as age, sex, laboratory findings, and prognosis were investigated.

Results: There were 33 cases (21.6%) of mycoplasma hepatitis. Their mean age was 4.4¡¾3.1 years old and male to female ratio was 1.2:1. It predominately occurred in spring (60.6%). Its clinical manifestation was nonspecific such as loss of appetite (72.2%), nausea(48.5%), vomiting (30.3%) and only 3 cases (9.1%) of hepatomegaly and no cases of hyperbilirubinemia, hypoproteinemia, hypoalbuminemia. Elevated asparate transaminase(AST) and alanine transaminase (ALT) level was 80.6¡¾31.3 and 159.6¡¾70.0 IU/L respectively. There were only 7 cases (4.6%)of ALT elevation among total patients and all recovered within 1 week. There was no statistical difference between hepatitis and non-hepatitis in terms of age, sex, admission days except season (spring).

Conclusion: This study showed that it was benign and prognosis was fair. The prevalence was not neglectable and hepatits should be considered when mycoplasma pneumonia has been found.
